Purpose:

This project provides outreach services, information and assistance, congregation of meals, and transportation services. The Putnam County Achievement Service offers just about anything seniors need, if not provided at the center, they would be connected to other agencies that do help with the specific need.

Remarks:

Putnam County Achievement Services is open from 8:00 AM to 3:00 PM Monday through Friday. This project is also a Community Focal Point. PCAS provides assistance with Medicare D and Illinois Cares Rx forms. Health promotion is also provided where the agency offers free blood sugar screenings every Tuesday from 10:00 AM to 11:30 AM, and blood pressure screenings every Thursday from 10:00 AM to 11:00 AM. The agency has a "Lending Closet" where they loan out medical equipment free of charge. In addition to stated services, PCAS also has an Alzheimer's Caregivers Support Group that meets on the third Wednesday of the month at 12:30 PM. The agency has two trained staff members to help with insurance problems and provide information on their Senior Health Insurance Program. The agency also holds a flu shot clinic. They refer people to other agencies and legal services, as well as connect them to heating and cooling assistance, home delivered meals, and any other services not provided at the agency.
